Implementation notes
- Items 3, 5, 6, 7, 9 and 10 are reverse keyed exactly as the official scoring worksheet specifies.
- The age question is required scoring context, not an eleventh scale item.
- The web product always uses the official self-administered lookup columns.
- Raw-total-to-score tables contain 41 entries each for raw totals 0 through 40 and were transcribed from the official standard-version scoring worksheet.
